Advanced Care Planning

Get the Care You Want
Having conversations about the “what ifs” is hard. But having a plan ensures you get the care you want.
In the event a patient cannot make their own medical decisions, a healthcare directive details the patient’s wishes and gives a trusted person the power to make decisions on their behalf.
CRMC offers support to help you complete a health care directive. To set up a free appointment, please call the social worker department at (218) 546-2357.
You can also download a healthcare directive form, get started and provide it to your care team. (Long Form) (Short Form)
